Abstract

Abstract: Introduction: Myelolipoma is a rare benign neoplasm of the adrenal cortex, formed by developed adipose tissue and hematopoietic components. It was first detected by pathologists Edgar von Gierke and Charles Oberling. Its appearance is usually with greater incidence between the fifth and seventh decades of life, predominating in the right adrenal gland. We present the case of a 62-year-old male who came to the hospital with abdominal pain in the right hypochondrium, showing as a tomographic finding a mass with heterogeneous lobulated edges of -90 to -5 HU that caused displacement of neighboring structures, suggestive of adrenal incidentaloma. In view of symptoms, he was taken to the operating table and the piece was removed. The results of the pathological anatomy concluded it as a myelolipoma. Objective: To review the historical evolution, clinical and imaging characteristics, and the therapeutic approach to adrenal myelolipoma, with emphasis on its current diagnosis and surgical criteria. Methods: A narrative review of the scientific literature on myelolipoma was conducted, including articles indexed in databases such as PubMed, Scopus, and SciELO. Epidemiological, diagnostic, and surgical aspects were analyzed, as well as reports of ectopic sites. Conclusion: Information on the aforementioned pathologists is scarce in the current literature, despite their important contributions to modern medicine. Information on the main adrenal incidentalomas is also scarce. This review is even more so regarding myelolipomas. This review highlights the historical evolution of this entity, its surgical relevance and the main approaches, as well as the imaging methods used for its diagnosis, thus providing further information for future research
