Abstract

Objetive: Compare the use of bipolar vessel sealing (LigaSure
precise®) with conventional knot tying in thyroid surgery.
Demonstrate the reduction in operative time and in blood loss in
thyroid surgery in patients admitted at surgical service II and III,
Hospital Domingo Luciani, Caracas.
Patients and methods: We made a descriptive, prospective
and comparative study, between august 2006 and august 2007.
Sample were patients with thyroid pathology. Twenty six with
conventional knot tying surgery and twenty one with LigaSure.
Results: Reduced intraoperative blood loss ( median range)
55 + 36cc (LigaSure group) vs 166 + 93cc (convencional knot
tying group) (p = 0,013). Shorter operative time 80 + 21 min
(LigaSure group) versus 98 + 26 min (convencional knot tying
group) (p = 0,001). No diferences between complications (
hypocalcemy, recurrent nerve palsy), use of drainage (latex, closed
aspirative drainage). Most frecuent pathology in the study
was multinodular goiter disease (61,7%). Most frecuent surgery
was Total thyroidectomy ( 51%).
Conclusion: LigaSure precise is safe in thyroid surgery, shortens
operative time and reduce blood loss comparing with conventional
knot tying surgery.
